Producer Agreements

Thank you for considering doing business with G. J. Sullivan Co. We are committed to providing excellent service. Quotes cannot be issued without all proper paperwork being completed and submitted for approval. Please submit the following paperwork. We will respond as soon as possible.

  • Current copy of E&O insurance declarations page
  • Current copy of agency as well as agent's license
  • Producer Packet (includes Producer Data Sheet, Producer Agreement, Payment Terms and W-9)

You will be prompted to submit your E&O declarations page and agent’s license electronically with your packet. If you prefer, you may submit your paperwork via fax. All completed paperwork must be submitted electronically or faxed to 714-876-2252. Providing all of the above information does not guarantee a producer agreement approval.
